بررسی عوامل اجتماعی و فردی مرتبط با تعداد دفعات اقدام به خودکشی در شهر مشهد با استفاده از مدل رگرسیون دو سطحی دو جمله ای منفی با صفر انباشته

چکیده

مقدمه: نرخ خودکشی جهانی حدود 6.5 در صدهزار نفر است، اما در برخی مناطق، به ویژه منطقه مدیترانه شرقی بالاتر است. در ایران، نرخ خودکشی در دو دهه گذشته به طور قابل توجهی افزایش یافته و به طور متوسط 9.9 در صدهزار نفر است. هدف این مطالعه بررسی ارتباط عوامل اجتماعی و فردی مرتبط با تعداد دفعات اقدام به خودکشی می باشد.

روش‌ها: یک مطالعه مقطعی در مشهد انجام شده که به بررسی تمام موارد ثبت شده تلاش به خودکشی در سال 2016 پرداخته است. داده‌ها از سوابق پزشکی اورژانس جمع‌آوری شد که شامل اطلاعات جمعیتی، روش‌های خودکشی و تلاش‌های قبلی بود. از یک مدل رگرسیون دو جمله ای منفی دو سطحی با صفر انباشته برای تخمین پارامترهای اجتماعی و فردی استفاده شد.

نتایج: در مجموع 2,666 مورد اقدام به خودکشی، زنان 64% از موارد را تشکیل می‌دادند. مسمومیت دارویی رایج‌ترین روش خودکشی بود. مدل رگرسیون دوجمله ای منفی دو سطحی نشان داد که زنان نسبت به مردان خطر نسبی کمتری برای تلاش‌های مکرر داشتند. علاوه بر این، سطوح بالاتر تحصیلات با کاهش خطر تلاش‌های مکرر مرتبط بود، در حالی که هیچ ارتباط معناداری در مورد تلاش‌های مکرر بر اساس وضعیت تاهل یافت نشد.

نتیجه‌گیری: نتایج این مطالعه ضرورت مداخلات هدفمند را که بر عوامل فردی و اجتماعی تمرکز دارند، برای کاهش مؤثر نرخ‌های خودکشی تأکید می‌کند. همچنین بر اهمیت افزایش آگاهی در مورد سلامت روان، به ویژه در میان گروه‌های آسیب‌پذیر مانند مردان و جوانان تأکید دارد.

Background and Aims: The global suicide rate averages around 6.5 per 100,000, with elevated rates observed in certain regions, particularly the Eastern Mediterranean. In Iran, suicide rates have significantly risen over the past two decades, averaging 9.9 per 100,000. This study highlights the complex nature of suicide behaviors, which are shaped by a community and individual factors.
Materials and Methods: A cross-sectional study was carried out in Mashhad, examining all recorded suicide attempts from 2016. Data was collected from emergency services and medical records, encompassing demographics, methods of suicide, and prior attempts. A two-level zero-inflated negative binomial regression model was employed to estimate both community and individual parameters.
Results: A total of 2,666 cases were analyzed, with females comprising the majority at 64%. The highest frequency of attempts was observed in the 20-30 age group. Poisoning emerged as the most common method, especially involving medications. A two-level zero-inflated negative binomial regression model indicated that women had a lower relative risk of repeated attempts compared to men. Additionally, higher educational levels were associated with a reduced risk of repeated attempts, while no significant correlation was found regarding repeated attempts based on marital status.
Conclusion: The study underscores the necessity for targeted interventions that focus on both individual and community-level factors to effectively lower suicide rates. It emphasizes the importance of raising mental health awareness, particularly among vulnerable groups such as men and youth.
