Metabolic dysfunction–associated steatotic liver disease, also known as non-alcoholic fatty liver disease, is the most common cause of liver disease worldwide. The term metabolically related fatty liver disease encompasses all forms of fatty liver disease that conform to the traditional view of nonalcoholic fatty liver disease, or NAFLD and shows a spectrum of liver diseases associated with insulin resistance. With the increasing prevalence of obesity, metabolic fatty liver disease is also increasing and has become a major clinical and economic challenge. This condition is now recognized as one of the most common reasons for liver transplantation in patients with liver disease. The association of metabolic fatty liver disease with obesity, metabolic syndrome, advanced age, and cardiovascular complications poses several unique challenges in the field of liver transplantation. In this article, challenges related to metabolic disorder-related fatty liver disease and its treatment are examined by reviewing articles published in PubMed, Google Scholar, ISI Web of Science, and Scopus databases.
