Identifying Community and Individual Factors Related to the Suicide Attempts in Mashhad through a Two Level Zero Inflated Negative Binomial Regression Model

Abstract

Background and Aims: The global suicide rate averages around 6.5 per 100,000, with elevated rates observed in certain regions, particularly the Eastern Mediterranean. In Iran, suicide rates have significantly risen over the past two decades, averaging 9.9 per 100,000. This study highlights the complex nature of suicide behaviors, which are shaped by a community and individual factors.
Materials and Methods: A cross-sectional study was carried out in Mashhad, examining all recorded suicide attempts from 2016. Data was collected from emergency services and medical records, encompassing demographics, methods of suicide, and prior attempts. A two-level zero-inflated negative binomial regression model was employed to estimate both community and individual parameters.
Results: A total of 2,666 cases were analyzed, with females comprising the majority at 64%. The highest frequency of attempts was observed in the 20-30 age group. Poisoning emerged as the most common method, especially involving medications. A two-level zero-inflated negative binomial regression model indicated that women had a lower relative risk of repeated attempts compared to men. Additionally, higher educational levels were associated with a reduced risk of repeated attempts, while no significant correlation was found regarding repeated attempts based on marital status.
Conclusion: The study underscores the necessity for targeted interventions that focus on both individual and community-level factors to effectively lower suicide rates. It emphasizes the importance of raising mental health awareness, particularly among vulnerable groups such as men and youth.
