Investigation of the causes of maternal near-miss at Shahidan Mobini Educational and Medical Center (2017-2021)

Abstract

Background:The maternal mortality index, as one of the most important global health indicators, is always of critical concern. Analyzing near-miss risk factors can contribute to improving maternal care. This study aimed to investigate the causes of severe obstetric complications among near-miss mothers at Shahidan Mobini Educational and Medical Center.
Materials and Methods:This retrospective, descriptive cross-sectional study was conducted on pregnant women who gave birth at Mobini Hospital (Sabzevar, Iran) between 2017 and 2021 and experienced severe pregnancy complications. Data collection tools included a demographic questionnaire and the World Health Organization clinical/intervention-based criteria for maternal near-miss cases. All delivery records were reviewed, and cases with severe complications were analyzed in terms of type, frequency, and causes. Data were analyzed using SPSS software version 26 with descriptive statistics.
Results: The mean age of mothers in this study was 29 ± 6.8 years. 230 near-miss cases out of 16,454 deliveries were reviewed. The rate of severe maternal complications was 13.98, and the maternal near-miss rate was 1.94 per 1,000 live births. The most common pregnancy-related risk factors for near-miss events were preeclampsia (38 cases, 16.5%) and gestational diabetes (12 cases, 5.2%).
Conclusion: Given the major risk factors, appropriate interventions for managing preeclampsia-eclampsia and its complications, as well as proper control of maternal gestational diabetes, are crucial in reducing severe maternal complications.
