علل اشتباهات دارویی و راهکارهای مقابله با آن در پرستاران و دانشجویان پرستاری

چکیده

مقدمه: اشتباهات دارویی بعنوان یک مشکل جدی در جهان محسوب شده و یکی از شایع­ترین خطاهای پزشکی است که امنیت و جان بیماران را تهدید می­کند. هدف از مطالعه حاضر بررسی علل اشتباهات دارویی و راهکارهای مقابله با آن در پرستاران و دانشجویان پرستاری می­باشد.  
مواد و روش­ها: این مطالعه از نوع توصیفی-تحلیلی بود که روی 327 نفر از پرستاران شاغل در بیمارستان خاتم الانبیاء (ص) و 62 نفر از دانشجویان دانشکده پرستاری و مامایی شهر زاهدان در سال 94 انجام شد و تمامی واحدها از طریق سرشماری وارد مطالعه شدند. داده­ها توسط پرسشنامه­ای که روایی و پایایی آن مورد بررسی قرار گرفته بود جمع آوری شدند. آنالیز اطلاعات توسط آمار توصیفی، آزمون تی مستقل و آنالیز واریانس یکطرفه با نرم افزار SPSS نسخه 16صورت گرفت.
یافته­ها: نتایج نشان داد که شایع­ترین علت خطای دارویی در پرستاران خستگی ناشی از بار کاری زیاد (8/97 درصد) و در دانشجویان پرستاری محاسبات دارویی اشتباه (4/77 درصد) می­باشد.
نتیجه گیری: بر اساس یافته­ها پیشنهاد می­شود که مدیران پرستاری کمبود نیروی انسانی را برطرف سازند. برگزاری کارگاه­های آموزشی در خصوص آماده ­سازی داروها، عوارض دارویی، اطلاعات دارویی و استفاده از کارت­های دارویی الکترونیکی نیز می­تواند در کاهش بروز اشتباهات دارویی مؤثر باشد.

Background and Aims: Medication errors are considered as serious and the most frequentworldwidemedical errorsthreatening patient’s safety and mayeven lead to their death. The purpose of this study was to investigate the causes of medication errors and  providingcounterstrategies to nurses and nursing students.
Materials and Methods: This descriptive – analytical study was conducted on 327 nursing staff of Khatam-al-Anbia Hospital and 62 intern students in faculty of nursing and midwiferyin Zahedan, Iran. Theenrollment was based oncensus in 2015. A valid and reliable questionnaire was applied to collect data. Descriptive statistics, t-test and ANOVA using SPSS16,on the other hand,were applied to analyze the data.
Results: The results showed that the most common causeof medication errors of nurseswasfatigue due toincreased workload (97.8%), and the most common onefor nursing students was calculation errors (77.4%).
Conclusion:Considering the results, it is recommended that nurse-managers sort out the human resource problems. Providingin-service workshops and educations about preparing medications, side-effects of drugs and pharmacological knowledge and using electronic medication cards could also result inmedication errorreduction.
