Background and Purpose: Health fulfillment depends on planning and attention to all factors influencing it. Oral dental health is one of the 11 slogans of 21st century, and one of the earlier PHC components. Also, DMFT is a simple, fast and reliable index in determining oral-dental health. Grade 3 of primary school is the bridging period between deciduous and permanent dental growth, characterized with mixed teeth making DMFT possible. Therefore, this study was conducted to determine the oral-dental health status of students using DMFT index in Gonabad, Iran. Material and methods: In this descriptive cross-sectional study, all grade 3 students of primary schools (529 students) were examined and interviewed for oral-dental health status under the supervision of a dentist. The study instruments were a checklist of DMFT index, a questionnaire, disposable mirror, and a special catather. Data were analyzed in SPSS with t-test, correlation coefficient and ANOVA. Results: 51.2% of the sample were male and 49.8% female. Also, 77.3% used tooth-brush and only 8.32% applied dental floss. Student dmft in research was 3.86±1.11, and DMFT was 1.04±.22, that showed no significant differences across gender. Students with no caries were 8.3%. One-way ANOVA showed a significant relationship between brushing frequencies and the index. Also, household and parental education indicated a significant relationship with DMFT. Conclusion: The results showed that oral-dental health status of grad 3 students of Gonabad primary school follow the nationwide pattern, and seems fairly optimal; also, CF has no significant difference with the national index but ranks average in relation with WHO goals. Scheduled efforts for KAP improvement, resource allocation, also revision in the administration, cost-effectiveness of some measures such as fluoride therapy, etc seem necessary
