چکیده
زمینه و هدف: استافیلوکوکها، باسیلهای گرم منفی خانواده انتروباکتریاسه، سودوموناسها از عوامل شایع عفونتهای خونی هستند. بسیاری از عوامل باکتریایی نسبت به آنتیبیوتیکهای مختلف مقاومت نشان میدهند. لذا هدف از این مطالعه بررسی الگوی حساسیت آنتیبیوتیکی در عوامل باکتریایی عفونتهای خونی جدا شده از بیماران بیمارستان شهدای خرمآباد در طی سال 1395 بود.
مواد و روش ها: تعداد 91 ایزوله باکتریایی از کشتهای خون در بیمارستان شهدای خرمآباد جداسازی و سپس به وسیله تستهای استاندارد آزمایشگاهی شناسایی شدند. تست حساسیت آنتیبیوتیک به وسیله روش دیسک- دیفیوژن Disk diffusion بر اساس CLSI انجام گرفت. نتایج با نرمافزار STATA-12 آنالیز گردید.
یافتهها: بیشترین ایزوله جدا شده از کشتهای خون اشریشیاکلی و استافیلوکوکهای کواگولاز منفی بودند. که بیشترین مقاومت در اشریشیاکلی به سفتریاکسون و جنتامیسین و در استافها به کلیندامایسین و سفوکسیتین بود که این موضوع حاکی از شیوع استافهای مقاوم به متیسیلین (MRSA)بود. همچنین کمترین مقاومت را به ایمیپنم داشتند.
نتیجهگیری: در این مطالعه نتایج جداسازی باکتریهای شایع در کشت خون با سایر مطالعات همخوانی دارد و بررسیهای الگوی مقاومت آنتیبیوتیکی گویای افزایش مقاومت دارویی در این باکتریها است. به علاوه بهترین آنتیبیوتیک برای درمان عفونتهای استافیلوکوک کواگولاز منفی ونکومایسین و برای سایر عفونتها ایمیپنم بود. به منظـور پیـشگیری از آلـودگیهـای متقـاطع، سیستمهای مراقبت و نظارت مؤثری برای کنترل عفونتهـای بیمارستانی ناشی از MRSA مقـاوم بـه چنـد دارو مورد نیاز است.

چکیده [English]
Background and Aims: Staphylococci, Enterobacteriaceae family, Pseudomonas and Enterococci are common causes of blood infections. Many of these bacterial factors are resistant to different antibiotic groups. Therefore, this study aimed to determine the antibiotic susceptibility pattern in bacterial agents of blood infections isolated from patients in Shohada Hospital in Khorramabad during 2016 - 2017.
Materials and Methods: : A total of 91 bacteria isolates were isolated from blood cultures in Khorramabad Shohada Hospital and then studied by standard laboratory tests. Antibiotic susceptibility test was performed by disks diffusion method according CLSI. Data were analyzed using STATA-12 software.
Results: The high frequencies were related to E. coli and Staphylococcus. E. coli showed the highest resistance to ceftriaxone, gentamycin and Staphylococcusto clindamycin, cefoxitin, this suggests an outbreak of methicillin-resistant staphylococcus (MRSA), and the least resistance to imipenem.
Conclusion: The results of this study were consistent with other studies, and antimicrobial resistance patterns suggest increase in these bacteria. In addition, the best antibiotic for the treatment of staphylococcal coagulase negative was vancomycin and for other infections was imipenem. In order to prevent cross-contamination, effective monitoring and surveillance systems are needed for controlling hospital infections due to multi-drug resistant MRSA.
