Introduction: Diabetes can have negative effects on public health, physical function, and
development of complications, psychological condition, and individual, family, and social
communication. Quality of life has been accepted as an effective aspect of patient care. Materials and methods: This descriptive study was conducted on 231 patients with diabetes
mellitus type 2 who visited the Sabzevar Diabetes Center. Subjects were enrolled in the study
using simple random sampling and the SF-36 questionnaire was filled for each subject. After
collection and classification, data was analyzed with descriptive statistics and analytical tests
using the SPSS software. Results: The average age of the subjects was 53 years and the average duration of illness was
7 years. The average general health score was below 4 (on a scale of 0-8) in 75% of subjects.
The general health score had an inverse relationship with number of children and a direct
relationship with education level (p<0.05).
Conclusion: Ambulation problems and physical and psychological difficulties were observed
more frequently in women than in men, whilst social difficulties were observed more
frequently in men. Education can play a big role in reducing the difficulties.

Introduction: Nowadays much attention is paid to the satisfaction of recipients of services.
The only effective method for guaranteeing the survival of organizations, reducing their
disadvantages, increasing their capabilities, and enhancing their qualitative and quantitative
improvements is to maintain customer satisfaction. Students are the major constituents of a
university and their points of view are considered as the key element in the success of the
institution. The aim of this study is to evaluate students’ satisfaction from the Education
Department of Sabzevar University of Medical Sciences.
Materials and methods: In this cross-sectional study, students in the second semester of
2007-2008 were recruited for evaluation. A questionnaire consisting of demographic data and
a Likert-scale evaluation checklist of the various sectors of the Education Department was
filled for the subjects. The results were then analyzed using the chi-squared and Fisher exact
tests using the SPSS software.
Results: Six hundred completed questionnaires were analyzed. Students at the associate
degree level reported higher satisfaction than bachelors students (p=0.007). The highest
satisfaction was observed among students of medical emergencies. In general, satisfaction
level of junior students was higher than seniors (p=0.001).

Evaluation of the awareness of chronic heart failure patients in Sabzevar Vasei Hospital regarding the exacerbating factors of their disease in 2009-2010

Introduction: Recognition of exacerbating factors is as important as underlying reasons in
the evaluation of chronic heart failure (CHF) patients. Relapse of CHF, further admissions,
and reduced life expectancy result from patients’ low compliance to treatment
recommendations. The aim of this study was to determine CHF patients’ awareness regarding
CHF exacerbating factors. The results of this study could be taken into account in therapeutic
and patient care programs. Material and methods: In this descriptive, cross-sectional study, 172 CHF patients admitted
to Sabzevar Vasei Hospital were analyzed. Data was collected by the researcher through an
author-made questionnaire and interviewing. Results were analyzed by the SPSS software,
using Student’s t-test and Chi-squared test at a confidence level of 95%. Results: The study results showed that only 3.16% (n=28) of CHD patients had a good
awareness of the exacerbating factors of their disease. Unawareness rate was 6.82% (n=142)
for infection, 3.84% (n=145) for pregnancy, and 1.83% (n=143) for fever as factors leading to
exacerbation of CHF. In this study, 7.65% of patients did not have adequate awareness
regarding fluid intake, and 89% were not aware of the recommended amount of food intake.
Conclusion: This study revealed that the awareness of CHF patients about exacerbating
factors of their disease is low. Noting the importance of these factors in relapse of CHF,
appropriate educations should be included in therapeutic and patient care programs in order to
reduce wasting time, hospital costs, and disease complications

Characteristics of sewage of Mobini Hospital, Sabzevar, Iran

Introduction: Hospital sewage, if not purified, can result in the spread of infectious diseases
and environmental pollution. This study aims to evaluate the qualitative and quantitative
characteristics of Sabzevar city Mobini Hospital sewage and to provide appropriate treatment
plans for this kind of sewage.
Materials and methods: The quality and quantity of crude sewage of Mobini Hospital was
evaluated in this cross-sectional study. Water consumption data was collected from water
bills. Per capita sewage production was calculated through identifying the waste production
ratio from consumed water. Twelve samples were collected and analyzed for biochemical
oxygen demand (BOD5), chemical oxygen demand (COD), sewage sludge treatment (SST),
total coliform (TC), and PH.
Results: Average water consumption and waste production per hospital bed were 569 and
512 liters per day and the waste production ratio was 0.9. Mean pH, BOD5, COD, and SST
were 8.2, 240 mg/L, 640 mg/L, and 291 mg/L, respectively. The coliform most probable
number was more than 1100 in 100 ml.
Conclusion: According to the findings of this study, a biological treatment method, such as
rotating biological contactor (RBC), should be used for hospital sewage, followed by
clarification and a chemical treatment due to the high level of COD.

Case report: The role of folic acid in the prevention of meningomyelocele

Introduction: Meningomyelocele is the most severe dysraphism of the vertebral column and
is described as bulging of the meninges and spinal cord from an osteal defect in the vertebral
column. Several risk factors, such as drugs, low socio-economic status, hyperthermia, genetic
factors, and dietary factors, including deficiency of zinc and folic acid, are considered to be
effective on the development of neural tube defects (NTD). Case report: This case is an infant boy, born via vaginal delivery and with gestational age of
39 weeks. In general examination, there was a significant 16-cm defect in lumbosacral region.
The normocephalic infant had dropsical eyes with a broad nasal bridge. He had a right club
foot, and examination of other organs did not reveal any abnormal conditions. Mother was 30
years old, gravida 3, with uncomplicated pregnancy and delivery, and with no history of drug
usage. Previous children were healthy and parents were not related. The important issue was
lack of folic acid and vitamin intake by the mother during the previous pregnancy. The infant
underwent meningomyelocele resection after 48 hours and was discharged after 4 days with
partial recovery. Results: Findings of this report indicate the importance of having a maternal diet with folic
acid for prevention of neural tube defects such as meningomyelocele. Among the risk factors
of NTD, much more emphasis is usually placed on factors like mother’s age, relative
marriages, medical history, and medication usage during pregnancy, while the easily
preventable nutritional deficiency of folic acid is often neglected. Therefore, giving advice
and education to mothers with or without a history of giving birth to a child with a neural tube
defect can largely reduce the incidence of these complications.

Introduction: Progress and development of information technology (IT) has formed a great
potential for improvement of quality and performance of health care services. With the
development of electronic facilities in Iran, there is a compelling need for electronic health
records (EHR). Advantages of EHR are better adherence of patients and health care workers
to preventive and treatment protocols, immediate access to laboratory and radiology results,
reduction in traffic, and improvement in service quality and decision making of the health
system stakeholders.
Materials and methods: The current study provides a review of the current literature and
attempts to identify the existing potential for EHR and its role in formation of an integrated
health information system of individuals, as well as its advantages.
Results: Implementation and commissioning of EHR requires high costs and a powerful
support. It also requires coordination and collaboration of all dependent and related section
managers.
Discussion and conclusion: Due to the changes in physician-patient relationships, patients do
not regard their physician as the only source of health information. Since patients frequently
need to use the services of multiple health service providers, the medical teams need to
exchange their information, and the insurance companies and health care managers need to
access comprehensive and up-to-date data regarding the population, it is necessary to move
toward EHR.
